I think maybe there was no device set for combat audio by the game and force switching it to something else, then back to the same device updated it on the game end. Windows > Sound > App Volume and device preferences > Find FFXIV and change the Output (top entry) from whatever it is set to to anything else, then back to what the correct device is.
Then on the cable I check listen to this device so I can still hear the audio. Then in OBS I have 2 sound inputs 1 for my mic and 1 for the virtual audio cable. If your audio goes out, change Equalizer Mode to "Standard" in System Settings. People were telling me that restarting my game was the only way to fix it, but I decided to try every other option in sound settings to try and kick it back in. It also isn't a driver issue, as other audio does not cut out, it's literally just discord. Randomly, but seems to be happening more and more frequently, audio from Discord will cut out.
